A RACI matrix maps tasks to team members with four role types: Responsible (does the work), Accountable (owns the outcome), Consulted (provides input before the decision), and Informed (notified after). It prevents confusion about who does what.
Every task needs at least one R and exactly one A. C and I are optional. Some tasks might have no one consulted or informed, and that is fine.
Responsible means you do the work. Accountable means you own the outcome and sign off. One person can be both R and A, but there should be only one A per task. Multiple people can be R.
